Custom Order Questionnaire - 2025PLEASE READ! THIS IS NOT AN ORDER. This form is to gather information regarding the project you are looking to have completed. This is to help insure that I AM able to complete your project - size, time & pricing - to your specifications. Once all details have been provided & I have done my my part to ensure I can provide you with a final product you will love, THEN an order will be placed & money will be handled. Minimum starting price is $20, with a minimum of three weeks to be completed - AFTER the order is officially placed. Orders $100 or less, payment is due up front. Orders $100 or more, $50 is due up front, the remaining due before shipment. Shipping cost will be included in the final price.
